1. Who Provides Your Insurance Policy?
The insurer of this Personal Accident Policy is Financial Insurance Company Limited, who provides and underwrites this policy, registered in England No. 1515187, limited by shares and whose registered offices are Building 11, Chiswick Park, Chiswick High Road, London, W4 5XR.
2. What Cover Do You Receive?
This policy covers you in the event of Permanent Total Disability, Loss of Sight in One Eye, Loss of Sight in both Eyes, Loss of Use of a Limb, Loss of Use of two Limbs, Loss of Hearing, Loss of Hearing in One Ear, Loss of use of a Shoulder, a Hip, Knee, Ankle, Wrist or Thumb, any Finger or Big Toe, any other Toe, Hospitalisation, Accidental Death
3. How Long Am I Covered For?
Benefits are payable if you have an accident after the start date of the cover and before the end date of the cover. Your Policy Document will provide you with full details of the benefits you are covered for. Your cover is for one month at a time as is automatically renewed every month until it is either cancelled or you miss paying three continuous monthly premiums or you die or permanently retire (whichever happens first).

5. What Cancellation Period Is There?
You may cancel your policy at any time by writing to the Insurer at the following address:

The Insurer may cancel your policy by sending a registered letter to you at your last known address, providing at least 30 days advance warning of the cancellation is given to you. If the policy is cancelled after you have made a valid claim this will not effect any benefits. You will not be entitled to a refund of premiums paid before the date of cancellation, unless you cancel within 30 days of receiving your insurance schedule and have not made a claim. If you do not cancel your policy within 30 days of the start date your policy will remain in force and may then only be cancelled by you or by us giving 30 days notice in writing.

7. What Are The Significant Or Unusual Exclusions And Limitations Of This Policy?
As with most insurance policies exclusions and limitations apply – the list provided below is not exhaustive, but highlights exclusions and limitations, which may be considered significant. For full details see the Policy Document (page 9)
We will not pay you benefits for an accident that is directly or indirectly the result of your suicide or attempted suicide, you using any type of alcohol or drugs unless under the advice of a doctor for a condition other than alcohol or drug addiction, if you die, suffer loss of a limb, loss of use of a shoulder, elbow, hip, knee, ankle, wrist, finger, thumb or toe, loss of sight, loss of hearing because of an accident, we will not pay you any benefits if any of them happen 12 months after the accident.
If the effects of an accident on you are made worse because you already had a condition, sickness, disease or injury then we will assess the effects that your condition, sickness, disease or injury has on your bodily injury and we will reduce your benefit by an appropriate amount to take this into account. 10. Financial Services Compensation Scheme
We are covered by the Financial Services Compensation Scheme. You may be entitled to compensation from the scheme if We cannot meet our obligations. This depends on the type of business and the circumstances of the claim. Insurance advising and arranging is covered for 100% of the first £2,000 and 90% of the remainder of the claim, without any upper limit. DEFINITION OF TERMS USED IN THIS POLICY
In this policy document and the Schedule the following words have the meanings given below. These words are highlighted by the use of
bold print.
- "Accident" means a sudden and unexpected event which happens after the Start date and results in Bodily injury.
- "Bodily injury" means injury to Your body (excluding sickness, disease or any naturally occurring condition or degenerative process) resulting from external violent and visible means.
- "Children/Child" means each of the Insured's children or legally adopted children.
- "Disability" means a state of incapacity resulting from an Accident.
- "Doctor" means a legally qualified medical practitioner other than You or a member of Your immediate family.
- "End date" means the date when Your insurance ends as set out on page 11 of this policy.
- "Hospital" means a lawfully registered establishment providing medical and surgical treatment and 24-hour a day nursing care for ill or injured people. This does not include a convalescent, self-care or rest home, or a department in a hospital which has the role of a convalescent or nursing home.
- "Hospitalisation" means staying in a Hospital on the advice of a Doctor because of an Accident.
- "Insured" means the person named in the Schedule.
- "Insurer" means Financial Insurance Company Limited. Registered office: Financial Insurance Company Limited, Vantage West, Great West Road, Brentford, Middlesex, TW8 9AG
- "Loss of hearing" means total and permanent loss of hearing in either one or both ears.
- "Loss of sight" means total and permanent loss of sight in either one or both eyes.
- "Loss of use" means total and permanent loss of use of a shoulder, hip, knee, ankle or wrist.
- "Loss of use of a limb" means total and permanent loss of use or loss by physical and permanent separation at or above the wrist or ankle.
- "Loss of use of a finger, thumb or toe" means total and permanent loss of use or loss by physical and permanent separation of the entire finger, thumb or toe.
- "Partner" means a person who is either a spouse/civil partner of the Insured or a person who has lived with the Insured permanently for 6 months or more'.
- "Permanent Total Disability" means a total and permanent Disability which medical evidence confirms will last for the rest of Your life and which stops You doing any paid job.
- "Premiums" are paid monthly by Direct Debit and include Insurance Premium Tax at the current rate.
- "Start date" means the date described in the Schedule.
- "United Kingdom" means England, Scotland, Wales, Northern Ireland, the Channel Islands and Isle of Man.
- "You or Your" means for:- (1) Individual cover – the Insured; or (2) Individual & Partner cover – (i) the Insured; (ii) the Insured's Partner; or (3) Individual & Children cover – (i) the Insured; (ii) each of the Insured's Children; or (4) Individual, Partner & Children cover – (i) the Insured; (ii) the Insured's Partner; (iii) each of the Insured's Children.
Your Schedule will show the type of cover chosen and in force.
ELIGIBILITY
An
Insured is eligible for cover if:
(i) the Insured is aged 18 or over and under 70 at the Start date; and
(ii) throughout the duration of the policy the Insured is living permanently in the United Kingdom for at least 40 weeks in any one year period commencing from the Start date and annually after this.
An
Insured's Partner is eligible for this insurance if they are aged 18 or over and under 70 at the
Start date and permanently living with the
Insured in the
United Kingdom. The
Insured's Children are eligible for this insurance if they are permanently living (excluding term-time) with the
Insured in the
United Kingdom and either under the age of 18 or under the age of 21 if still in full-time education.
If occasional or temporary absences mean that
You are going to be outside the
United Kingdom for a total of more than 12 weeks in any one year period commencing from the
Start date and annually after this, then
Your cover outside the
United Kingdom will stop on the last day of the twelfth week.
If
You wish to extend
Your cover for occasional or temporary absences (which means
You will be outside the
United Kingdom for more than 12 weeks in any one year period commencing from the
Start date and annually after this) then please write to the
Insurer with full details. The
Insurer will then decide whether
Your cover can be extended and if it can
You will be sent written confirmation which
You should place with this policy document in a safe place.
You will only be eligible for insurance cover under one Nationwide Personal
Accident Insurance at any one time.

Notes to Benefits Table:
Your Schedule will show the Benefit Level chosen and in force.
You may change
Your Benefit Level at any time.
All benefits are paid to the
Insured or the
Insured's legal personal representative.
You cannot transfer or sell
Your rights or benefits under this policy.
The benefits set out in the Benefits Table will go up by 5% of their starting amounts on each anniversary of the
Start date.
When
You reach
Your 65th birthday
You will no longer be eligible for
Permanent Total Disability and all other benefits will be reduced by 50%.
All benefit calculations will be made at the date of the
Accident.
If
You have an
Accident You should be put under the care of a Doctor as soon as possible as failure to do so may affect
Your claim.
Where it is proven that the effects of an
Accident on
You are made worse because
You already had a condition, sickness, disease or injury then the
Insurer will assess the effects that
Your condition, sickness, disease or injury has on
Your Bodily injury and the
Insurer will reduce
Your benefit by an appropriate amount to take this into account.
If
You claim for any of items 1-7 in the Benefits Table and then die within 12 months as a result of the Accident the following benefits will be payable:
- if the benefit You receive for the Accident is less than the death benefit, the Insurer will pay the difference between the two.
- if the benefit You receive for the Accident is greater than the death benefit, no further payment will be made.
A. Permanent Total Disability
Assessment of
Your eligibility for
Permanent Total Disability benefit will be delayed for 12 months from the date
Your claim was received by the Insurer so that an independent
Doctor of the
Insurer's choice can assess
Your claim. However, if it is shown by medical evidence that the
Disability is permanent then the Insurer may at their discretion pay a benefit before the end of the 12 months.
B. Loss of Limb
If
You have already claimed under item 2 of the Benefits Table then
You will not be eligible to claim under items 5, 6 or 7 for that limb. If
You have already claimed under items 5, 6 or 7 this amount will be deducted from the benefit payable under items 2(i) or 2 (ii) of the Benefits Table.
C. Hospitalisation
The daily rate benefit:-
(i) is for each complete 24 hour period in Hospital; and (ii) is subject to a maximum of 180 days in Hospital for each Accident.
WHAT IS NOT COVERED
A. The
Insurer will not pay
You benefits for an
Accident that is directly or indirectly the result of:
(i) war, invasion, acts of foreign enemies, civil war, rebellion, or whilst You are on naval, military or airforce duty, service or operations;
(ii) riding on a motorcycle as a driver or passenger;
(iii) rock climbing, mountaineering of any type or parachuting;
(iv) You working as a paid professional sports person;
(v) You competing in any race other than on foot or whilst swimming;
(vi) flying except as a fare paying passenger;
(vii) Your own self-inflicted injury, suicide or attempted suicide;
(viii) Your own illegal acts;
(ix) You taking any type of alcohol or drugs unless under the advice of a Doctor for a condition other than alcohol or drug addiction;
(x) radiation or contamination or the effects of radiation.
B. If
You die, suffer
Loss of use of a limb, Loss of use of a finger, thumb or toe, Loss of use of a shoulder, hip, knee, ankle or wrist,
Loss of sight or Loss of hearing because of an
Accident the
Insurer will not pay
You any benefits if any of them happen 12 months after the date of the
Accident.
POLICY LIMITS
The maximum benefits payable for all claims are:-
Individual Cover
The
Permanent Total Disability benefit and the maximum
Hospitalisation benefit.
Individual & Partner, Individual & Children or Individual,Partner & Children Cover
For each person the
Permanent Total Disability benefit and the maximum Hospitalisation benefit up to an aggregate maximum amount of twice the
Permanent Total Disability benefit.
HOW TO MAKE A CLAIM
If
You wish to make a claim under this policy, please ask for a claim form from:-

The claim form must be filled in and sent to the
Insurer at the above address. All claims must be submitted within three calendar months of the date of the
Accident or as soon as possible, as failure to do so may affect
Your claim.
All certificates, information and evidence required by the
Insurer in substantiation of a claim must be provided at
Your own expense and must be on the form provided by the
Insurer. In the case of
Bodily injury You shall as often as the
Insurer reasonably requires agree to medical examinations at the
Insurer's expense in connection with any claim.
We support the Association of British Insurers claim code.
You can be provided with a copy of this document upon request.
DISHONESTY
Any fraud, mis-statement or concealment either in the application on which this insurance is based, or in relation to any other matter affecting this insurance, or in connection with the making of any claim, shall render this insurance null and void and any benefits paid under a claim are to be returned to
Us.
PREMIUMS
The premium shown on Your schedule is payable monthly and includes Insurance Premium Tax. If there is any future change in the rate of Insurance Premium Tax the premium will be automatically adjusted. Any alteration in the rate of tax will be announced publicly.
If You do not pay Your first monthly premium Your policy will be void from the Start date. Your monthly premium will go up by 5% of its starting amount on each anniversary of the Start date.
RENEWAL OF INSURANCE COVER AND POLICY REVIEW
Your policy is automatically renewed at the end of each complete calendar month after the
Start date.
The
Insurer may vary or amend the terms and conditions of
Your policy at any time provided at least 30 days written notice is given to
You prior to any alteration taking effect.
WHEN DOES YOUR PROTECTION END?
A.
Your policy ends automatically as soon as one of the following happens:
(i) the Insured dies;
(ii) the Insured reaches 75 years old;
(iii) the date on which the Insurer pays benefits which together with any previous payments equals the maximum benefits payable as set out in Section 5 on page 6 of this policy;
(iv) the date You do not pay a monthly premium when due;
(v) the date You or the Insurer cancels Your insurance as set out in Section 11 on page 8 of this policy.
B. If
You have Individual &
Partner, Individual &
Children or Individual,
Partner &
Children cover
Your protection will also end if:-
(i) You are a Partner of the Insured when You reach 75 years old;
(ii) You are a Child of the Insured when You reach 18 years old or,
if You are still in full-time education, You reach 21 years old;
(iii) You stop permanently living with the Insured.
Please write and tell the
Insurer when B.(i), (ii) or (iii) happens so that
Your policy details can be changed accordingly.
CANCELLATION
You may cancel Your policy at any time by writing to the Insurer at the following address: Financial Insurance Company Limited, Building 11, Chiswick Park, Chiswick High Road, London, W4 5XR. The Insurer may cancel Your policy by sending a registered letter to You at Your last known address, providing at least 30 days advance warning of the cancellation is given to You. If the policy is cancelled after You have made a valid claim this will not affect any benefit payments. You will not be entitled to a refund of premiums paid before the date of cancellation, unless you cancel within 30 days of receiving your insurance schedule and have not made a claim. If you do not cancel your policy within 30 days of the start date your policy will remain in force and may then only be cancelled by you or by us giving 30 days notice in writing.
NOTICE
Unless otherwise stated in this policy any notice given under this policy shall be in writing and sent by post and shall be deemed to have been served one day after posting if sent by first class post, or three days after posting if sent by second class post.
DATA PROTECTION
You understand and give explicit consent that the sensitive health and other information that You provide about yourself will be used by Financial Insurance Company Limited, its agents and associated companies, other insurers, regulators and public bodies (including the police) and agencies to process this insurance and any other insurance, handle claims and prevent fraud. This may involve the transfer of such information to other countries (including those which have limited or no data protection laws). Financial Insurance Company Limited have taken steps to ensure that Your information is held securely.

If You buy insurance in the UK, You can chose which law will apply to the Policy. English Law will apply to Your Policy unless We make a written agreement saying otherwise. The information supplied in Your Policy document will be in English.
